What is on it

  • A box for every day of the month, 1 to 31. Tick each day school happened.
  • An hours-by-week table, Monday to Saturday, so the monthly total adds itself up.
  • A box for subjects covered and notes such as holidays, sick days and trips.
  • A total instructional hours box and a parent or guardian signature line.
  • Undated: write the month in the heading and print a fresh copy each month.

How to fill it in

  1. Write the month and the child's name (or "all") in the heading.
  2. Keep it somewhere you look every day, such as the fridge or the front of your planner.
  3. At the end of each school day, tick the date and write the hours in that week's row.
  4. Add holidays, sick days and field trips to the notes box as they happen.
  5. On the last day, add up the hours, sign it and file it with the rest of the year.

Rules on what to keep vary by state and by country, and some places ask for none. Check your own: HSLDA keeps a state-by-state summary of US homeschool laws, and your state's education department has the official wording. This sheet is a record-keeping tool, not legal advice. It is secular and works with any curriculum. Families who report per child may prefer one copy each.
